If you are seeking to add a new telephone or change existing telephone services at your site, you must work with your ITS Field Operations Director for your Campus (or ITS representative at College Office). Although the work is coordinated through ITS staff, charges are incurred by the campus.
Your request should include all necessary information, including site location for the install, building and room number, the name of the employee (for voice mail setup), and a description of special features requested. You will be notified about how many days it will take to schedule the installation.

Requests for moving equipment must include site location/building/room number where the phone is located, inventory number (5-digit handwritten number under the phone), name of employee who is moving, telephone number and site location/building/room number to which the phone is moving. If there is a phone in the location where the phone is moving, please supply that phone number and its inventory number as well. You will be notified about how many days it will take to schedule the move.
Requests to remove a phone must include site location/building/room number, telephone number to be removed, inventory number (5-digit handwritten number under the phone), and the date by which the phone must be removed. The phone should be removed with 5 days, after ITS submits the request to the service vendor.
Requests to upgrade a phone must include the site location/building/room number of the phone to be upgraded, the inventory number (5-digit handwritten number under the phone), explanation of upgrade, type of key set, and the phone number if you are keeping the same number after the upgrade.
Wiring requests for new telephone or data lines and moving existing telephone or data lines must go through the ITS department. Requests must be approved by your Business Manager before work can be scheduled. All phone installations are debited to your budget.
Installation costs per line, voice or data, are determined by the distance to the nearest wiring closet. Installation cost includes labor, wire, jacks, connectors, termination and testing. For specific estimates of wiring costs, contact Jim Pifko at firstname.lastname@example.org.
Requests for Voice Mail should be made as you are submitted requests for a new phone, or changes to a phone.
Changes to Voice Mail accounts are necessary when an employee is assigned to a phone number, which is equipped for voice mail from a previous employee. Contact the Help Desk with your request to reset the voice password. Once the password has been reset, see the Voice Mail Documentation for setting a new password and programming your message.
